1. Agreement to Terms
By accessing or using exclusivmed.com (the "Site") or booking any service from [LEGAL ENTITY NAME — e.g., ExclusIV Medical Services, LLC] ("ExclusIV", "we", "us"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree, do not use the Site or our services.
2. Services Offered
ExclusIV provides mobile IV therapy, intramuscular injections, telehealth consultations, private-event services, travel medical support, and related wellness products. Specific services, pricing, and availability are described on the Site and are subject to change.
Services are offered only to adults age 18 or older, or minors with the consent of a parent or legal guardian. We reserve the right to refuse service to anyone for whom treatment would be medically inappropriate or unsafe in our clinical judgment.
3. Medical Disclaimer
Information on the Site is provided for general educational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment. 4. Booking and Payment
- Bookings are confirmed only after payment is processed and we send a confirmation message.
- Prices listed on the Site are in U.S. dollars and may be subject to applicable taxes and travel fees, which are calculated at checkout.
- Payments are processed by our third-party providers (Square and PayPal). Their terms govern the payment transaction.
Cancellations and rescheduling
You may reschedule or cancel an appointment at no charge up to [24] hours before the scheduled time. Cancellations made less than [24] hours in advance, or no-shows, may be subject to a fee of up to [100%] of the service price. If our team is unable to attend due to weather, equipment failure, or other reasons within our control, we will reschedule at no cost or refund in full.
Refunds
Services already delivered are non-refundable. Unused product purchases may be returned, unopened, within [7] days for a refund less any applicable shipping or restocking fee.
5. Consent to Treatment
Before each appointment, you will be asked to review and sign our Consent for Treatment and, where applicable, our Self-Injectable Medication Liability Statement. You acknowledge that all medical procedures carry inherent risks and that no specific outcome is guaranteed.
6. Eligibility and Honest Disclosure
You agree to provide accurate and complete health information, including current medications, allergies, conditions, and pregnancy status. You assume all risk arising from incomplete or inaccurate disclosure.
7. Accounts
If you create an account on the Site, you are responsible for maintaining the confidentiality of your password and for all activity under your account. Notify us immediately of any unauthorized use.
8. Acceptable Use
You agree not to:
